Shazam Magic, based out of Frederick, MD, asks for a booking form to allow prospective clients to sign-up for magic shows online. Form made with PHP and includes a confirmation page and a thank you page. An automatic, formatted e-mail also gets sent to both the client and the company with all the information from the form.

2008 Updates: Form now interacts directly with booking database, entering prospective clients into the Magician's gig-database, sorted by date of show. Form also includes a CAPTCHA to prevent spamming.
